PVS server shows offline in the PVS console, although Streaming service is running on occasion:

An error in event viewer:

Event ID: 11 – Undefined database error/PVS server status is "Down"

Resolution

Complete the following steps:

Restart Citrix PVS Soap Server
Check Supported Databases for Virtual Apps and Desktops AND Citrix Provisioning (PVS)

Note:
SQL Server Native Client support is removed from Citrix Provisioning 2109 and later and not required anymore

Microsoft OLE DB Driver is now installed with specific Citrix Provisioning versions

Check root and intermediate certificate of SQL server, from the affected PVS server.
If required request a certificate from your local domain Certificate Authority and apply it to the SQL server hosting the PVS database.
Create SPN hostnames for the SQL server's domain account in the AD (SPN hostnames should get created automatically when you installed SQL Server role)
Re-run the Citrix Provisioning configuration Wizard again.

If the issue still occurs: collect logs and upload to MyCitrix Account
