[ADC] ICA sessions are NOT sent to ADM with "Skip Code [169]"

Description
You may observe some ICA sessions are displayed in GUI > Configuration > Citrix Gateway > ICA Connections, but not in ADM HDX Insight. NetScaler (ADC) ns.log displays the following error:
ICA_RECORD: Skipping ICA flow: ...... Skip Code [169]
Resolution
Focus on the HA failover events, it has a lot of root causes and solutions. Please refer to: Understanding the causes of failover
Problem Cause
The Skip Code [169] means NetScaler ADC doesn't want to report this session because of:
| 169 | NS_ICA_ERR_HIGHER_RECONSEQ | Higher CGP resumption sequence number from peer post reconnects |
Error 169 usually appears for ICA re-connect sessions after ADC HA failover.
