When you login to ADC we can confirmthat upgrade of ADC is done ,ns.log file :-
This is an upgrade from NetScaler NS12.0 build 57.24 to build 58.18
Resolution
Upgrade ADM to 12.0.59
Problem Cause

When upgrading ADC from ADM it's stuck while one ADC upgrades and reboots and ADM reports ADC not reachable
