You may encounter this error if you are using a controller which was already a part of the SQL server where we are attempting to create a new database
Environment

Citrix cannot guarantee that problems resulting from the incorrect use of Registry Editor can be solved. Use Registry Editor at your own risk. Be sure to back up the registry before you edit it.
Resolution
You may remove the controller's machine account login from SQL server and then attempt to create site again.
Delete the machine account in the SQL Server:
Security -> Logins:
Domain\DDCHostname
Delete the machine account in the SQL Server:
Security -> Logins:
Domain\DDCHostname
Problem Cause
Account already existed in the DB, so it cannot initialize the schema creation process
